Completing this form allows us to assist you in temporarily turning off water service to your home, as well as restoring service when the time comes.
This form is also used to request changes to your mailing address, name, or if a tenant change has occurred at a rental property.

ACH Payment (Automatic Deduction) Set-Up Form

Pay your monthly water bill directly from your checking or savings account with an automatic deduction. Enroll by providing this completed form along with your bank account information. Documents that will provide the necessary bank information include: a voided check, deposit slip, or other bank-provided documentation that shows both the account and routing numbers for the account you wish payment to be deducted from. Once your application has been processed, the automatic deduction will occur on the 15th of each month. Forms received after the 10th of the month will be processed with the first payment being deducted on the 15th of the following month.
